History: I was walking around all of a sudden I found a Demon Charm. I put it on and I transformed into this form. I suddenly heard a voice in my head. "You have discovered your true form..." The voice said. "Put this on whenever you wish to transform..."

Are you absolutely sure you want to do this? You only get a max of 5 forms....
